# Risk Register Template (AvaSkill Academy — Project Management) A risk register is the living list of everything that could go wrong (or right) on your project, with a plan for each. Review and update it regularly — a risk register written once and forgotten helps no one. ## How to score a risk - **Probability:** 1 (rare) to 5 (almost certain) - **Impact:** 1 (negligible) to 5 (project-killing) - **Score = Probability × Impact** (1–25). Act first on the highest scores. ## Register | # | Risk description | Prob (1–5) | Impact (1–5) | Score | Response strategy (avoid / mitigate / transfer / accept) | Planned action | Owner | |---|---|---|---|---|---|---|---| | 1 | | | | | | | | | 2 | | | | | | | | | 3 | | | | | | | | | 4 | | | | | | | | | 5 | | | | | | | | ## Response strategies — quick guide - **Avoid:** change the plan so the risk cannot happen. - **Mitigate:** reduce the probability or the impact. - **Transfer:** shift the impact to a third party (e.g. insurance, contractor). - **Accept:** watch it, but take no action now (have a contingency plan for high scores). ## Review log | Date | Changes made | Reviewed by | |---|---|---| | | | |
